Event Liability Waiver and Photo/Video Release

By registering for and participating in the Witness the Fitness Fundraising Event (the "Event") on August 9, 2025, at Indiana Christian Academy, located at 432 W 300 N, Anderson, IN 46012, participants agree to the following terms:

Release of Liability: Participants acknowledge that the activities involved in the Event, including but not limited to weightlifting (squat, bench press, and deadlift), carry inherent risks of injury. By participating, you voluntarily assume all risks and responsibilities associated with these activities. Participants agree to hold harmless, indemnify, and release Witness the Fitness, Madison County Fire Rescue House, Indiana Christian Academy, event organizers, volunteers, sponsors, affiliates, and their respective agents, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, or causes of action resulting from injury, loss, or damage arising out of participation in this Event.

Medical Condition: Participants affirm that they are in good health and proper physical condition to safely participate in the activities. Participants acknowledge that it is their sole responsibility to consult with a physician prior to participation if they have any pre-existing health conditions or concerns.

Photo and Video Release: Participants acknowledge and consent to being photographed and/or videotaped during the Event. Witness the Fitness and Madison County Fire Rescue House reserve the right to use these photographs and videos for promotional purposes, including but not limited to websites, social media, advertising, and future event promotions, without additional compensation or permission.

By registering for and participating in the Event, you confirm that you have read, fully understand, and voluntarily agree to the terms and conditions of this waiver and release.

Minors under the age of 18 require parental consent on the day of the event to particpate.
